How to use

1. Locate a provider in your area by calling the 800 on the back of your Master Healthcare card or go to the Master Healthcare provider link and find a participating provider in your zip code area.

2. Present your card at your appointment with the doctor, dentist, optometrist, pharmacy or other healthcare provider.

3. If the healthcare provider has any questions, refer them to the numbers on the back of your card.

4. When you visit is completed the provider will apply your savings and collect the payment for services.

NOTE: To receive the negotiated discount rate, you must pay in full at the time of service.
